Start small enough to follow through.
Choose one priority with the team and, where appropriate, the people receiving care. Use this record to organise the conversation from the concern to the next decision. Urgent concerns still need action through the service’s normal safety and safeguarding procedures.
The method develops our earlier 90-day improvement review and the programme’s governance feature. Novaspec’s supplied approach brings an emphasis on listening, a jointly shaped plan and continued support. The working record itself is Care Circle’s editorial aid, rather than a prescribed CQC form or a compliance assessment.
Your working record
One improvement, from concern to review.
Use brief planning notes and roles. Keep identifiable care information in your organisation’s approved records. Notes stay in this page until you download them or leave.
Describe one concern and the difference people should experience.
Record any immediate protection and the relevant escalation already taken.
Name the accountable role, the next action and its due date.
Identify learning, supervision, resources or specialist input needed.
Plan how to include people receiving care, staff and other relevant voices.
Choose proportionate evidence that could show progress or expose a problem.
Set the review date, reviewer and decision: continue, adjust, escalate or close.
At the review, make a decision.
Look at the evidence alongside the experience of the people affected. Ask what changed, what remained difficult and whether the response created another problem. Make disagreement visible, and record what still needs an answer.
Then decide whether to continue, adjust, escalate or close the action. Closing an action should include a reason and a proportionate account of what has been checked. A completed form alone cannot demonstrate a sustained improvement.
Where specialist support can help.
A provider may know what needs to change but need help with training, supervision, systems or an unresolved regulatory question. Novaspec describes one-off reviews, mentoring, governance support and continued work alongside teams. Discuss the work needed, the relevant expertise and how progress would be reviewed.
For legal, clinical or safeguarding decisions, use the appropriate professional advice and statutory routes. This record helps organise a service-level review; it does not determine an individual’s treatment, rights or care arrangements.

Tell Novaspec about your service, its priorities and the support already in place. Ask how the team would get to know you, agree the work and stay involved as it develops.
